What to do if patients are not feeling the Nitrous effectively

Possible causes:

  • The mask is not fitting the patients faceThe one way valve in the nasal cone is missing or stuck in an open position, failing to close upon patient inhalation
  • The breathing bag has a hole, tear, or slit in itLoose hose or connector in the breathing circuit
  • The Room Air Safety Valve is stuck openThe nitrous oxide concentration is not set correctly for the patient

  • Some people self-medicate before going to the dentist

What to do?

  • Check the system to ensure no leaks in the bag or hoses
  • Verify that the tubes are clear and that there are no kinks
  • Verify that the valves are in place in the nasal hoods
  • Don’t forget about the fit of the mask
  • Ensure breathing pattern is duplicated via the breathing bag
  • Request patients to reveal any medications or alcohol they have taken prior to treatment

Do you service the RA mixer and associated equipment? How often?

Nitrous Oxide Mixers and associated regulators must be maintained as per manufacturers recommendations and accreditation requirements.

This is normally 12 months inspection / calibration checks and 5 yearly overhaul of mixers.

How do patients feel on nitrous oxide?

  • Nitrous oxide is called “laughing gas”, because it produces a “happy state”.
  • You might experience a sense of relaxation, “dreaming” feeling and may smell a faint sweet scent.
  • Nitrous oxide also numbs pain and may help suppress a gag reflex.
  • People under nitrous oxide may feel “tingly” on lips, fingers and legs, flushing in the face as well as sweating.

Is nitrous oxide sedation safe?

Nitrous oxide is possibly the safest sedative in dentistry and has been used for over 100 years. Nitrous oxide is analgesic, works quickly, and doesn’t cause allergies. The level of sedation can be controlled by the concentration given to the patient by the dentist, and sedation can be terminated and reversed anytime by flushing the patient with oxygen. Nitrous oxide sedation is a form of conscious sedation, meaning that patients will remain fully conscious during the treatment and responsive to questions while staying in a comfortable, relaxed state.

Who will need nitrous oxide sedation?

For some people, visiting the dentist may be one of their most stressful experiences. Nitrous oxide, for the majority of patients, is an effective sedative agent used to decrease anxiety, provide a state of relaxation and anaesthetize (numb) the teeth, though local anaesthetic is still required. With its relaxing effects, nitrous oxide allows children and adults to put up with the sounds and sensations of dental treatment.

How many patients can usually be treated before D cylinder refills are required (Mobile System)?

Example: Divide the contents of cylinder by the flow rate used “D” Oxygen holds 1500 litres, at an average flow rate of 6 lpm would give approx 250 minutes of gas per. cylinder “D” Nitrous Oxide holds 3200 litres, at an average flow rate of 6 lpm would give approx. 530 minutes of gas Average use normally estimates 4 x Oxygen to 1 x Nitrous Oxide

Is it necessary to Autoclave the complete patient circuit?

It is imperative to autoclave any reusable nasal hoods and the Scavenging Cones after every use.

The Patient Circuit is totally autoclavable and maintained within each surgeries protocol.

Air Liquide Healthcare also has fully disposable circuits available.

What is meant by exhaust gas scavenging?

The removal of expired gases from the patient and operatory, to the atmosphere, usually via the high or low suction vacuum controllers.

How do you tell what is left in a Nitrous Oxide Cylinder?

Nitrous Oxide is a liquid, which boils off to a gas under pressure.

When the liquid level is too low the pressure contents gauge on the regulator will drop quickly.

This indicates the cylinder is due for refilling.

Alarm requirements for piped systems?

All piped gas systems must comply with AS2896, where automatic manifolds are installed.
